The code 780.39 is a sign or symptom catagory. (I don't have my book with me) The code 250.00 means diabetes mellitus. This has 9 forth digit headings, ie diabetes and 2501_ diabetes w/ketoacidosis ,2504_, DM w/renal complications, etc. The 5th code indicates further catatgorization ie; 0= AODM (adult onset diabetes mellitus), and 1=IDDM (insulin dependent diabetes mellitus) and 2=AODM uncontrolled, and 3=IDDM uncontrolled. So 25063 would be Insulin dependent diabetes with neurological complications. 25002=simply is uncontrolled adult diabetes (non-insulin). As for OV EST PT and low and mod, that is referring to PROCEDURE codes found in the CPT book NOT ICD-9. Hope this helps.
