I have been electroncially billing my Dr.office psychiatric visits and almost all of them have been rejected, especially codes 90808 and 90804 can anyone tell me do I need to use a modifier on these codes or do medical records need to be submitted. Any input as to why would greatly be appreciated by me. Thank You
